MRT tragedy: 2 companies to be blacklisted?

KUALA LUMPUR, Sept 19, 2014:
MRT Corp has identified negligence as the main factor behind the deaths of three Bangladeshi MRT workers last month and plans to blacklist the two responsible companies.
The two companies are Aecom Perunding Sdn Bhd and Syarikat Muhibah Perniagaan dan Pembinaan Sdn Bhd.
MRT Corp chief executive Datuk Azhar Abdul Hamid said the Aug 18 incident was a tragedy that could have been avoided if the two companies had laid out a proper standard operating procedure (SOP) and provided sufficient supervision on the site.
“The 350-tonne concrete span had toppled due to the absence of shims to support the viaduct deck span, leaving it in an unstable position.
“The construction workers were installing parapets on the imbalanced span, and as there was no SOP given to them, they ended placing 13 parapets on only one side of the span, instead of both.
“This had caused the span to topple along with the three construction workers,” Azhar explained at a press conference.
He said in the investigation report, the corporation had identified the personnel and the organisations responsible for the incident, that took place at a MRT worksite near the former Rubber Research Institute in Kota Damansara.
